How to Apply

Step 1: Submit Your Application

Please note that you must be licensed within the U.S. or Canada to be eligible for the program.

Step 2: Confirm Dates

Once dates are confirmed, submit this form within one week. You will be sent a pre-signed Application for 60-Day Temporary Permit Certificate to use if you do not have an NC permit.

Step 3: Make a Payment

Once dates are confirmed, please make payment within five business days.

There are two payment options for our vet program, which cover up to four days of training:

  • $150 - Vets working for nonprofit spay/neuter clinic
  • $350 - Any other vet

Please note that if you have to cancel your training, we may not be able to refund your payment if we cannot reschedule the block, though we will make every effort to do so.

Step 4: Apply for Temporary State License (if applicable)

If appropriate, complete and submit the Application for 60 Day Temporary Permit Certificate at least 60 days prior to scheduled training, but not before your training has been scheduled (the NC Board will not reissue if dates change). This should be sent to the North Carolina Veterinary Medical Board. The application must also include:

  • $100 check or money order.
  • Passport photo taken within six months.
  • Transcript from college/school of veterinary medicine.
  • License verification from state where currently licensed.
  • Copy of your Social Security card.

Step 5: Submit Liability Insurance

Submit your proof of liability insurance.
